CONSENT TO PARTICIPATE
Thank you for participating in this important survey.
The purpose of this survey is to understand your experiences of working in healthcare at this point in time. This survey will explore the impact of various occupational stressors and highlight current challenges and useful supports. The data from this survey will be used to develop an organizational wellness and resilience plan aimed at supporting staff at Sanford Health.
The report will also look at key strengths and challenges of the organization and will provide practical recommendations to enhance the work environment for all. Confidentiality is critical and the report will in no way permit identification of any given individual.
Participation in this survey is strictly voluntary, and there will be no penalties or negative consequences should you choose to not participate. Confidentiality will be ensured during the collection of this information and participant anonymity will be maintained throughout the process of survey completion, data collection, and report preparation.
The data will be coded and stored under Françoise Mathieu’s direction. Françoise Mathieu is a Registered Psychotherapist in the Province of Ontario and the Executive Director of TEND Academy Ltd. The data will be safeguarded, such that only Ms. Mathieu or other qualified researchers, approved by her, shall have access to it.
Analyses of the data, research reports, and all other uses of the data conform to the Ethical Standards and Principles of Responsible Research as defined by the College of Registered Psychotherapists of Ontario.
